During registration, it is necessary to indicate whether the patient will need an interpreter during the visit:

- sign language

- English language

- Ukrainian language

 

You can book a visit:

  • by phone: tel. 81 718 21 21. When calling the registration office, please have your PESEL number and e-referral code ready (if required). All calls are registered.
  • using the Patient Assistant on the website. You should send us an application with information on which clinic the patient wants to register for. Contact details must also be provided. Our consultant will contact the patient within 2 working days.
  • in person or through a third party. In such a case, you should take a ticket from the ticket machines located at the entrance, selecting the clinic to which the person wants to register. Then you have to approach the window above which the number from the ticket will be displayed.
  • Via Internet. You must register yourself via e-registration. Please remember to enter a valid phone number when registering.

Working hours of individual registrations

The Main Registration is open from Monday to Friday, from 7.00-14.35

The registration of the Lung Diseases, Respiratory Allergy and Oncological Clinic is open from Monday to Tuesday from 7.30-15.00, Wednesday at 8.30 - 16.00, Thursday - Friday 7.30-15.00

Registration of the Neurosurgical and Rehabilitation Clinic: Monday - Friday, 7.30-15.00

Where as soon as possible?

You can always unsubscribe from the waiting list at one facility and join the waiting list at another. However, you cannot register a referral in two clinics at the same time.

The order in the Waiting Queue

When determining the order in which health services are provided and placed on waiting lists, the medical category (urgent or stable case) shown on the referral is of great importance. The referring doctor qualifies for a given medical category. If you have special rights to use health care services out of turn, it is necessary to inform the registrar about it and present the appropriate document. In this case, we will try to provide you with the service on the day of notification or, if this is not possible, we will set a different date outside the order of admissions resulting from our waiting list.

Providing medical records regarding Outpatient Specialist Care to patients

REQUEST

1. We provide the medical history at the request of the patient, his/her legal representative or a person authorized.

2. Authorization for a statutory representative or a person authorized by the patient must be in writing.

3. You can submit a request for access to documentation:

-in person, during registration hours,

DATE LIMIT

-Medical records are made available/provided to patients as soon as possible.

-The employee accepting the request will inform you about the date when the records are ready to be picked up.

RECEPTION

You can pick up a copy of your medical records:

-at the Registration at the window during working hours,

-by sending by registered mail via Poczta Polska,

-by sending by e-mail, the file is encrypted with a password.

We record each provision of documentation in the "Register of Providing Medical Records", therefore we will ask you to confirm its receipt. We also ask for an ID card to carefully check the details of the person who comes to pick up a copy of the documentation.

FEE

-The first copy of the medical records is made available to the patient by the medical entity free of charge.

-For making another copy of the medical history, the patient pays a fee in accordance with the price list in force at IPTH No. 4 in Lublin.

Appointment at a specialist

INSURANCE

On the day of each visit, we check whether you have the right to health care services financed from public funds, through the electronic verification of beneficiaries (e-WUŚ).

If the e-WUŚ does not confirm your right to health services, you should present a document confirming your health insurance or sign a Declaration on the right of the beneficiary to health care services. FILE FOR DOWNLOAD

If you want to visit a doctor and you do not have insurance with the National Health Fund, we will ask you to pay the fee at the cash desk located in the Main Registration, at window no. 1 according to the price list (located at this link). We only accept cash.

First appointment

1.The first appointment to a specialist takes place on the basis of a referral issued by a doctor who has signed a contract with the National Health Fund (with the exception of gynecology and oncology clinics).

2.If you are under specialist treatment until its completion - you are not obliged to periodically renew the referral.

3.An update of the referral for specialist treatment is necessary when the patient has not started treatment for the last 730 days. A new referral is also necessary when the reason for issuing it, given in the previous referral, has changed.

4.If you have not registered in person, be sure to go to the registration window to confirm your personal data.

5.You should bring the results of tests that may affect the doctor's diagnosis.

6.At least once a year, you will receive a certificate for the referring doctor, which will allow you to continue the pharmacological treatment ordered by the specialist. On the basis of the certificate, the primary care physician will issue a prescription for medications you take on a regular basis.

7.If there is a need to continue treatment, the doctor will set the date of the next appointment or refer you to registration.

Follow-up appointment

Remember to confirm your arrival each time by entering your PESEL number on the ticket machine. The system will direct you to the appropriate office and the doctor will be informed that the patient is waiting outside the door.

Doctor's absence

If during registration you agreed to be contacted by us and send notifications, we will inform you about the doctor's absence by phone or via SMS.

Tests ordered by a specialist

When ordering the test, the doctor will inform you about the method of registration and the place where you should go to perform the test.

-The result of the test will be sent to the referring doctor, who will explain it and give it to a patient during the next appointment.

-If the doctor referring you to the examination does not immediately set the date of the next visit, immediately after arranging the date of the examination, call the registration desk at the telephone number 81 718 21 21.
