Independent Public Hospital No. 4 in Lublin  For the patient       

Independent Public Hospital No. 4 in Lublin is the largest highly specialized, teaching health care facility in Lublin. There are 25 wards in our hospital, with about 800 patients every day. Independent Public Hospital No. 4 in Lublin serves as a Trauma Center for the entire Lublin macroregion. Its task is to treat patients with severe multi-organ injuries. It also functions as a Cerebrovascular Accident Treatment Centre.

Below we present the most important information about the hospital's infrastructure, as well as important information for hospital patients.

Patient rights

 

The patient has the right to:

be provided with health services that meet the requirements of current medical knowledge,

obtain information about his/her health,

maintenance of professional secrecy,

expressing consent to the provision of health services,

get respect for intimacy and dignity,

access to medical documentation regarding patients’ health and the services provided,

filing an objection to an opinion or a doctor's decision,

get respect for private and family life,

pastoral care,

storing valuables in a deposit.

Admission to hospital. Necessary documents

 

A patient admitted to the hospital should have:

  • one of the following identity documents: ID card with PESEL number or passport or driver’s license. In the case of a child up to 18 years of age it can be a school ID or other document confirming identity. In the case of a child up to 3 months old who does not have a PESEL number, a document with the PESEL number of the parent or person obliged to register the child for health insurance should be presented.
  • referral from a doctor or dentist or feldsher (except for emergency medical services).

In emergencies or when, due to health conditions, it is not possible to submit a statement, the patient is admitted despite the lack of confirmation of the right to free services.

A document confirming the right to free healthcare services or a declaration should then be submitted: within 14 days from the date of commencement of the provision of healthcare services or within 7 days from the date of completion of the provision of the service (if the service was provided in a hospital ward).

A patient entitled to health care services under the provisions on coordination (EU patient) should have the following upon admission to the hospital:

- passport or other identity document

- referral (except emergency admissions),

- document confirming insurance, e.g.:

European Health Insurance Card (EHIC) or EHIC Replacement Certificate or confirmation issued by the Provincial Branch of the National Health Fund or E112/S2 form.

In the absence of confirmation of the right to free healthcare services, the patient is charged with the costs of the services provided.

Upon admission, hospital employees verify the patient's eligibility for free healthcare services through the Electronic Verification of Beneficiaries' Eligibility system. If the entitlement is not confirmed in the system, the patient should present a document confirming the right to free healthcare services (e.g. a decision on the health benefit, a certificate from the employer) or submit a statement on the right to free healthcare services.

What you should bring to the hospital

A patient referred for planned treatment in a hospital should bring: personal toiletries, cutlery, pajamas or nightgown, bathrobe, towel, underwear and slippers.

If a patient is admitted to the hospital in an emergency (health and life threatening conditions), the above-mentioned personal items should be delivered to the patient by the family or actual guardian.

The patient should give clothes and shoes for storage in the hospital clothes depository.

Money and valuables can be deposited in the hospital deposit and received against receipt on the day of discharge. The hospital is not responsible for valuables and money retained by the patient, not transferred to the hospital's deposit.

Mobile phones, portable computers and other electronic equipment may be used in patient rooms by patients only with the permission of the chief or his deputy, as such devices may interfere with the operation of electronic medical equipment.

General daily schedule in the hospital ward

Before breakfast, the temperature, pressure, pulse and the collection of samples for analytical tests are carried out. It is also advisable that each patient performs a morning toilet before breakfast. Mobile patients use the bathrooms on their own, while those who stay in beds are assisted by the professional staff of a given ward.

Ward rounds

Time between 8:00 a.m. and 12:00 is intended for ward rounds and performing procedures ordered during the round. During this time, patients stay in the ward. Each time the patient leaves has to be reported to the duty nurse or ward nurse.

Meals

Each patient receives at least 3 meals a day: breakfast at 7:30-8:00, lunch at 12:00-13:00 and dinner at 17:30-18:00. The attending physician prescribes an appropriate diet. All meals for patients are prepared in the hospital kitchen under the supervision of qualified dietitians. In addition to therapeutic diets, the hospital also offers a vegetarian, vegan and gluten-free diet. You can also get drinks (tea, boiled water) in the ward kitchen throughout the day.

Smoking is prohibited throughout the hospital!

Consumption of alcohol in any form is strictly prohibited throughout the hospital!

Visits

Patient visits take place on all days of the week, at the times indicated in the regulations, usually from 10:00 to 22:00. It is advisable to limit patient visits during ward rounds. Visitors should leave their coats in the cloakroom located in the hospital lobby (near the entrance). We ask that no more than two people participate in the visit at a time. Due to the risk of infection, visits by children under 7 are not recommended.

Due to the serious condition of patients, the doctor is entitled to suspend or limit visits to a given hospital room. In the event of an epidemiological threat, it may also be necessary to suspend patient visits in a given ward or even in the entire hospital for a limited period of time.

Providing information about the patient's health

Information about the patient's health is provided by a doctor to a person authorized in writing by the patient or his/her legal guardian. Nurses provide information only on nursing and care activities undertaken in relation to the patient and information what should be carried out so that the patient’s health condition continues to improve.

General information on providing treatment

Hospital at ul. Jaczewskiego is equipped with modern medical equipment that allows you to conduct complex examinations and analyzes. The equipment also makes it easier to make an accurate diagnosis of your health condition and choose the most effective treatment methods. Our staff strives to provide patients with the best professional care, bring relief to those suffering and create an atmosphere in which they would feel good and safe.

In order to secure the patient's professional matters, on the second day after arriving at the hospital, at the patient's request, a notice of admission to treatment may be sent to the workplace.

Patients staying in the hospital for less than 2 weeks receive a sick leave on the day of discharge. If the duration of the stay is longer, the doctor treating the patient issues a sick leave every 2 weeks.

Other necessary certificates can be obtained through a clerk of a given ward or directly at the Department of Patient Documentation and Medical Statistics (1st floor, room 142).

Organization of post-hospital care

The post-hospital care organization nurse and social worker

The post-hospital care organization nurse and the social worker help patients and their families to organize post-hospital follow-up care, if necessary. They propose and familiarize them with possible solutions in this area. This applies to e.g. complete the documents necessary to place the patient in a Social Welfare Home, a Care and Treatment Facility, organize home care, establish insurance, etc.

  Lonely, homeless, abandoned by their families and those in a difficult life situation, as well as families of patients requiring further care can count on help of the post-hospital care organization nurse and social worker.

Service points

 

Dressing room

We ask that all visitors to our hospital patients leave their outer coats in the cloakroom at the main entrance to the hospital before entering the wards. Other changing rooms are located within the hospital, i.e. in the PET, Pneumonology building, at the entrance to the Physiotherapy Department, at the entrance to Specialist Clinics and in the Operating Block. Cloakrooms are free of charge.

Library

The branch of the Municipal Library, stocked with many interesting books, is open every day except weekends and holidays. For patients who have to stay in bed, the librarian brings books to choose from to the wards. The library also subscribes to illustrated monthlies and weeklies. Information about the location and opening hours of the library and the reading room are available in the so-called nursing points in the wards.

Cafeteria

The cafeteria is located in the main building, in the middle part (zone 1B) on the 4th floor. It offers lunch sets, dishes by weight and drinks at an affordable price. The cafeteria is open from Mon.-Fri. at 8:00-15:00, Sat-Sun at 12:00-15:00.

Post office

The Polish Post office is located in the main building of the hospital, in its central part (zone 1C), on the ground floor, in the corridor leading to the Departments of Neurosurgery and Rehabilitation. You can send letters, parcels and money orders at the branch. The facility operates Mon.-Thurs. at 8:30-14:30, Fridays 11:00-20:00 with a break from12:00 to12:30. Letters, parcels, etc. come to the Hospital Office and are immediately forwarded to patients.

Press Shop - Kiosk

The press shop is located in the main building of the hospital, on the first floor, on the side of the Department of Electrocardiology and Cardiology departments (zone 3). Open on Mon-Fri 7-20, Sat. 7-19, Sun. 8-18

Bank

Cash desk of Bank PEKAO S.A. is located in the main building of the hospital, on the first floor, on the side of the Department of Electrocardiology and Cardiology departments (zone 3), near the press shop. The bank is open from Monday to Friday from 8:00 to15:00

ATM

ATM of Bank PEKAO S.A. is located in the main hall near the entrance to the Hospital (zone 1, first floor).

Medical shops

Specialist shops with medical supplies are located in two places: in the lobby at the entrance to the main building of the hospital, in the corridor leading to the "Groszek" grocery store on the ground floor of the main hospital building.

Taxi

It is possible to order a taxi through the porter's lodge at the entrances to the hospital

Parking

When entering the hospital premises, you can use the car park supervised by the Medical University of Lublin. You can enter the car park by collecting a parking ticket at the entrance. Drivers can pay the fees upon leaving the car park employee or on their own at the machine located in front of the main entrance to the hospital.

Store

On the lower ground floor of the main building, opposite the building of the pneumology ward (zone 2, lower ground floor), there is a grocery store "Groszek" open from Mon to Fri  from 7:00 to 9:00; sat. 8:00-18:00 Sun. 8:00-16:00.

Buffet/food corner

The buffet run by the hospital kitchen is located in the underground part, in the corridor between the main building of the hospital and the building of Specialist Clinics (zone 6, lower ground floor). It is open from Mon.-Fri. at 9:00-15:00. The offer includes hot dishes, sandwiches and bread, drinks, salads.

Coffee points

Takeaway coffee shops are located in two locations: in the central part of the main hall, on the first floor of the main hospital building (zone 1, first floor, high ground floor), on the high ground floor of the main hospital building, next to the post office.

Pastoral care

During their stay in hospital wards, the hospital provides patients with the right to pastoral care. The patient may at any time summon a clergyman of his/her denomination: patients do it in person, with the help of relatives, or they can ask a nurse to help in that matter.

The Roman Catholic Hospital Chapel is located between the main building and the building of the Departments of Neurosurgery and Rehabilitation (the so-called high ground floor), where every day at 7:30 and 19:00, and on Sunday at 9:00 and 15:00 the holy mass is celebrated by the chaplains of our hospital.

Each patient also has the opportunity to confess, receive Holy Communion and the sacrament of the sick in all wards during the so-called pastoral round.
